Save Money on Dental Care -- Special Rates and Discounts

Now you and your family can save on dental care expenses! The Unicare 100 Series offers you access to one of the largest networks of dental care providers, with more than 17,000 dental providers and specialists nationwide.

As an CHAA member, you and your dependents will receive special rates on specific preventative and diagnostic services that include oral examinations, radiographs and certain treatment (please see the list of scheduled benefits below) and a 15% discount on all other services If you see a network specialist, you will receive a 15% discount on all services.

Simply present your CHAA Membership Card to the provider and receive your discounts!

Saving Your Life -- The Gateway Medicard

Each year millions of people receive emergency medical care for injuries or illnesses. Emergency service providers may be unfamiliar with you and your health history. In an emergency, getting your health information to medical personnel as quickly as possible may be critical. Your Gateway Medicard allows you to keep your medical profile accessible all the time.

The Gateway Medicard is a laminated card that contains your medial information. You simply fill out a Personal Medical Profile, which is photographed on microfilm and laminated into a durable plastic card. Medical personnel can easily read this card with a standard magnifying glass (routinely carried by medical professionals).

As an CHAA member, you may order one free medical card per account per year. In order to ensure you medical data is current, every 12 months you will receive a reminder to update your medical card information. Should you need to update your card more often, you can do so for only $5. You can also order cards for other family members for just $5 each (similar cards cost $8 - $20 from other sources).

Lost Or Stolen Key Service

Who hasn't lost their keys at one time or another? And it always happens at the most inopportune time, causing you hassle and frustration! Now some of the inconvenience caused by lost or stolen keys is lessened. CHAA members will be reimbursed as follows for lost or stolen keys:

• For replacement of your lost or stolen keys - $15
• For a locksmith to open your car or home lock if your keys are lost or stolen - $35
• For a lock smith to change your car or home lock if your keys are lost or stolen - $50
• For the deductible amount under your primary insurance covering items stolen from your home or car, if your home or car is broken into bCHAAuse your keys are lost or stolen - $250
